By using my services, you agree to these terms. You can also read my Privacy Policy.
1. Who I Am and How I Can Help
I, Annette Clipsham, am your psychologist. That means I am responsible for your care. If you sign this agreement, you agree to work with me.
2. Privacy – Keeping Your Information Safe
I take your privacy very seriously. Since I run a virtual clinic, I collect and store your personal and health information digitally. This helps me:
Book your appointments
Keep medical records
Process payments and Medicare claims
I follow strict ethical rules to keep your information safe. You can read my Privacy Policy for more details.
3. No Reports for Court Cases
I do not provide reports for legal matters, like family court or insurance claims, unless I am required to by law.
4. Parenting Plans
If you have a parenting plan, the parent who books the session must follow it. I do not share session details with other parents or people unless you ask me to in writing.
5. Sharing Information with Others
If you want me to share information about your treatment with someone else (like a doctor or teacher), you must give me written permission. However, if Medicare or another group is paying for your sessions, I must send progress reports to your doctor or the funding group. By agreeing to these terms, you accept this.
6. My Training and Supervision
I meet regularly with senior psychologists (supervisors) to keep learning and improving. Sometimes, I may discuss my work with them, but I do not share your name or personal details.
I may also ask to record a session for training. You can say no to this at any time. If a session is recorded, it is safely stored and deleted after my training is complete.
7. Contacting Me Between Sessions
I am not able to answer phone calls or emails between sessions. If you need to reach me, please email annette@annetteclipsham.au.
If you request a phone call or a detailed email response, you will be charged my standard hourly rate. My fees are listed here.
8. Reports and Forms
If you need a report, letter, or application form, please give me at least 3 weeks' notice. These will be charged at my hourly rate.
Terms and Conditions for Psychological Assessments
1. Purpose of Assessment
Based on your information, I have determined that this assessment is reasonable and necessary for your needs.
2. Providing Accurate Information
It is your responsibility to provide accurate and up-to-date information.
If any details are incorrect or incomplete, please inform me as soon as possible.
Inaccurate information may lead to an unsuitable assessment recommendation.
3. Cancellations & Rescheduling
If I need to cancel or reschedule an appointment, I will give you at least 24 hours' notice whenever possible.
If you need to cancel, please refer to my Cancellation Policy for details on fees.
4. Assessment Fees & Payments
Each session will be invoiced on the day of the assessment. Payment is due within 7 days.
Questionnaires will be invoiced after the initial clinical interview, when they are sent for completion.
The assessment report will be invoiced after the final assessment session and must be paid in full before the report is written.
Payments can be made via direct deposit or credit card (see my Fees & Payment Policy for details).
5. Report Completion & Feedback
Your report will be completed once full payment is received, and a feedback session will be scheduled within 3 weeks.
6. Diagnosis & Clinical Outcomes
Payment for an assessment does not guarantee a diagnosis.
Diagnoses are made based on evidence and clinical guidelines.
Some assessments may result in a different diagnosis than expected, or no diagnosis at all.
You are paying for the assessment process and my clinical expertise, not a specific outcome.
